Bernardes venceu as eleições presidenciais de 1 de março de 1922, obtendo 466.877 votos contra 317.714 votos dados a Nilo Peçanha, em uma eleição que dividiu o país: Rio Grande do Sul, Bahia, Pernambuco e Rio de Janeiro apoiaram Nilo Peçanha e os demais estados deram apoio à candidatura Bernardes.
Antes da eleição, Bernardes teve que enfrentar o rumoroso caso das "cartas falsas" atribuídas a ele e que denegriam o ex-presidente Hermes da Fonseca.
Seu vice-presidente foi Estácio Coimbra que substituiu Urbano Santos, vice-presidente eleito, também em 1 de março de 1922, e que faleceu no dia 7 de maio de 1922, antes de tomar posse.
O descontentamento com a vitória de Bernardes e com o governo de seu antecessor, Epitácio Pessoa, foram algumas das causas do chamado Levantre do Forte de Copacabana, primeira ação do movimento tenentista. Bernardes teve que fazer frente à coluna Prestes, movimento tenentista que percorreu o país pregando mudanças políticas e sociais e que jamais foi derrotado pelo governo.
