Painted on this utility box in black and white are two big human eyes with long lashes looking up and to the right. The eyes are placed in such a way that they each wrap around onto two sides of the box. The "front" face points northwest. Though it's not obvious or, perhaps, intentional, the glance is towards an optometrist practice on Eddy Street.
Its background of multi-colored, random geometric shapes outlined in white covers every surface.
The southwest side includes the phrase: "PROVIDENCE THE CREATIVE CAPITAL" in 2-inch black bubble letters. This labels it as part of the Art Transformer Project, organized by the City of Providence Department of Art, Culture, + Tourism (AC+T).
