Das erste Stadtspital geht auf das 14. Jahrhundert zurück und stellte seit der Gründung der Universität Innsbruck im Jahre 1669 einen fundamentalen Teil der von Anfang an eingerichteten medizinischen Fakultät dar, bis diese 2004 zur eigenständige Universität erhoben wurde. Der erste Lehrstuhl für Chirurgie in Österreich wurde 1733 eingerichtet. 1742 schließlich verpflichtete die Reform Maria Theresias die Professoren dazu, die ärztliche Betreuung des Stadtspitals zu übernehmen.

Seit der zweiten Hälfte des 19. Jahrhunderts befindet sich die Uniklinik auf einem 90.000 m² großen Areal im Westen der Stadt gegenüber dem Hauptgebäude der Universität, auf dem bis heute alle Instituts-, Verwaltungs- und Forschungsgebäude angesiedelt sind. Zwischen 1885 und 1887 wurden die ersten klinischen Pavillons des „Neuen Stadtspitals“ errichtet (Verwaltungsgebäude, Chirurgie, Frauen- und Hautklinik).
