Votivní kapli nechala v dobe moru roku 1715 postavit hradištská vojenská posádka v prostoru mezi vnejším a vnitrním opevnením pred Kunovskou bránou, na nynejším námestí Palackého. Vysvecena byla 16. srpna 1716 a pozdeji byla na vrcholu štítu osazena plastikou svatého Šebestiána . V lednu 1969 byla kaplicka vyzvednuta, posunuta o nekolik metru jihovýchodním smerem na zvýšený terén a do konce roku 1971 celkove zrenovována. Poslední rekonstrukcí prošla v roce 1997.
Duvodem a impulsem k premisteni kaple byl zamer rozsireni kolem vedouci komunikace v souvislosti zvyseni kapacity autobusoveho nadrazi, které v te dobe bylo situovano na Palackeho namesti.
Pripravne práce byly zahajeny pocatkem rijna 1968 pod vedenim Ing. Josefa Sevely. Sto tunova kaple byla zvednuta do vysky 1,5 metru, pred tim byla vymenena trouchniva podlaha kaple a nahrazena 20 cm silnou armovanou deskou. K vlastnimu presunu byly pouzity specialni transportni voziky – hydraulicke tanky.
Presun byl zahajen 6. ledna 1969, kdy byla kaple presunuta o dva metry, pokracoval 7. ledna, kdy byla kaple posunuta o 7 metru (smerem od silnice k hotelu Grand). Dne 9. ledna 1969 byly práce dokonceny a kaple usazena na nove vybudovanou zakladovou desku.
Presun provedla firma Transfera podle patentu dr. Ing. J. Wunsche, Csc. (projektant dostavby sportovní haly v UH) a Ing. Sulaka. Jednalo se o první presun stavby v republice, nasledoval presun dekanskeho kostela Nanebevzeti P. Marie v Moste.

Votive chapel was at the time of plague in 1715 Hradištská build a garrison in the space between the outer and inner bailey Kunovský the gate, the current Palacky Square. Consecrated was 16 August 1716 and was later fitted with a shield on top of the sculpture of St. Sebastian. In January 1969 the chapel was picked up, moved a few yards southeast of elevated terrain and the end of 1971, total renovated. Last underwent reconstruction in 1997.
The reason and the impulse to move the proposal to extend the chapel around the communications manager in the context of increasing the capacity of the bus station, which at that time was located at Palacky square.
Preparatory work started in early October 1968 under the supervision of Ing. Joseph Sevel. One hundred ton chapel was lifted to 1.5 meters, before it was exchanged rotten floor chapel and replaced by 20 cm thick reinforced slab. The actual transfer was used specifically carts - hydraulic tanks.
Move started 6th January 1969, when the chapel was moved about two meters, he went on the 7th January, when the chapel was moved to 7 meters (away from the road to the Grand Hotel). 9 January 1969 the work was completed and established the chapel built on the new base plate.
Move performed by Dr. transfer according to the patent. Ing. J. Wunsch, Csc. (Designer completion of sports hall at UH) and Ing. Sulak. This was the first transfer of buildings in the country, followed by transfer of the Dean's Church of the Assumption of the Virgin Mary in Most.
