O actual Hospital de São José sucedeu ao antigo Hospital Real de Todos os Santos, que teve o seu arranque a 15 de Maio de 1492. Contou com a presença do Rei D. João II, na altura do seu quadragésimo aniversário, apenas três anos antes da sua morte em Alvor.
A sua inauguração ocorreu nove anos mais tarde, no reinado de D. Manuel I em 1501. A obra esteve a cargo do Mestre das Obras do Reino, o arquitecto Diogo Boitaca. Situava-se no Rossio, ocupando a área da actual Praça da Figueira
Resultado da fusão de pequenos hospitais a sua nomenclatura ficou determinada pela denominação comum aos que se juntaram como - de Todos os Santos (Omnia Sanctorum). Foi também "Hospital Real", e simultaneamente Hospital dos Pobres. As letras O e S mantém-se até hoje no símbolo que representa a instituição.
Conta na sua Igreja com pinturas de Fernão Gomes em dourado.
A 27 de Outubro de 1601 deu-se um dos grandes incêndios que destruiram quadros dos Reis de Portugal e as pinturas a fresco maneiristas. Outro incêndio deu-se a 10 de Agosto de 1750 e o terceiro e último, a 1 de Novembro de 1755 no incêndio resultante do Terramoto de Lisboa. A ironia foi a de que o Hospital Real de Todos os Santos foi destruído no seu próprio dia, o dia de Todos os Santos.

The current St. Joseph Hospital, succeeding the former Royal Hospital of All Saints, who had his start May 15, 1492. With the presence of King John II, at the time of his fortieth birthday, just three years before his death in Alvor.
Its inauguration took place nine years later, the reign of King Manuel I in 1501. The work was directed by the Master of the Works of the Kingdom, the architect Diogo Boitaca. It was located on the Rossio, occupying the area of contemporary Praca da Figueira
Result of the merger of small hospitals to its nomenclature was determined by the common name for those who joined as - All Saints (Omnia Sanctorum). It was also "Royal Hospital", and simultaneously Hospital of the Poor. The letters S and remains today the symbol that represents the institution.

On 27 October 1601 there was a major fire that destroyed frames of the Kings of Portugal and the Mannerist frescoes. Another fire occurred at Aug. 10, 1750 and the third and last to November 1, 1755 resulting in the burning of the Lisbon earthquake. The irony was that the Royal Hospital of All Saints was destroyed in his own day, the Feast of All Saints.
