From reference[1]:

"The memorial to the Great War records the more than 100 Guy’s men who lost their lives in that conflict. The Portland stone arch was designed by the Hospital architect W J Walford, unveiled in 1921 and restored and rededicated in 1994, when it was moved to its present location in the park. The reverse carries the names of Second World War losses."

It hasn't always been a free standing arch. According to reference [2] it used to connect two of the buildings in the hospital.
