Dumb Ways to Die (GC4W8Q7) by Fakari Mama

Background

I subscribe to the "Geocaching Puzzle of the Day" blog (click) where I receive a new puzzle cache to solve each day. I also troll Groundspeak (click) for interesting puzzle to try and solve, and maybe adapt for releases on the Island. Bottom line -- I end up solving some very interesting puzzle caches that I'll never get the chance to visit or adapt for one reason or another (puzzle won't work for local coords, graphics too hard to duplicate, etc). Then I recalled an interesting "Tribute Series" from NB's jim52 where he simply had you solve another cacher's puzzle to get the coordinates for a locally placed cache! The original puzzle CO gets the recognition (scorn?) for his/her creativity and you get the and exposure to a great puzzle -- win-win! Soooo, welcome to the Kudos Series where I pay tribute to someone else's great puzzle caches. I take no credit other than having solved them, often with the help of others -- as you may end up doing, too!

NOTE

Difficulty rating is based on original CO's rating; Terrain rating is based on local cache placement, as is the hint.
