This is right near 5th Street Roundabout. Go up the dirt roads, and it's off the trail/road just a tad.
This is another easy cache, quick to find. Accuracy was 18 Feet, Elevation 4718. Rating is 2 because there are several dirt roads you could take. See spoiler for easy access. Might want a truck with a little clearance, since the road is rutted, but 4x4 not required. Short walk but all uphill!
