You are looking for a drinking bottle attached by a brass clip to a chain set in a bucket of concrete. Please bring the bottle to the surface to retrieve the micro cache from within and to sign the log. We don't want the cache to get wet now do we? ;-) Don't forget you've got to attach it back to concrete block again.
***WARNING*** Diving is a safe sport when learnt properly. Please ensure you make an adequate dive plan and dare I say it, risk assessment. If in any doubt please don’t dive for this cache; go away and train some more. This is a difficulty 5 for a reason and not just because it's underwater. Keep it safe and enjoy both sports.
